The goal of Geriatric Care Management is to plan and manage the complex and changing needs of older adults and their families, with the expectation of maximizing the highest quality of life possible.

Geriatric Care Managers are qualified to assess the biological, psychological and social needs of older adults and others with chronic needs. We formulate care plans and can recommend appropriate resources and support for our clients.
The Geriatric Care Manager functions as an advocate, liaison, and/or support system to the client and their family members, providing ongoing screening, monitoring and follow-up to closely address any changes.
