- Facilitating and mediating communication between patients, physicians and families
- Medication set-up and monitoring
- Referrals to appropriate community resources (adult day care, home delivered meals, etc.)
- Adult residence, assisted living, and skilled-nursing facility placement, including well visits to ensure a positive adjustment to a new senior living environment
- Helping families and seniors find the best assisted living facility to meet their specific needs
